When selecting a plastic surgeon, patient frequently turn to online reviews.  We would like to warn our prospective patients that recently   someone  started writing negative reviews about our practice on various websites.  This poster under different user names creates  posts  with with false and misleading stories accusing us of causing different rhinoplasty complications.

        I have been performing rhinoplasty for over 10 years. Rhinoplasty is the most commonly performed procedure in my practice and I do this surgery every day. Over 90% of my patients are happy with their results. My practice is busy and based on numerous referral from my patients and colleagues, including their family members and friends. I have performed rhinoplasty on many of my own family members, employees, colleagues, and celebrities. Having performed numerous rhinoplasty procedures, we have been able to improve closed rhinoplasty techniques over the years  to produce better cosmetic and functional results.
Despite our high satisfaction rate,  occasionally we have patients who are not  happy with their results. Every busy rhinoplasty surgeon has patients who are unhappy with their rhinoplasty results. By now, I have re-operated or consulted on patients from most busy plastic surgeons in the country and many patients from abroad. Many unhappy patients actually have good noses and not every patient can be made happy.
       I do my best to understand what kind of result the patient is looking for and discuss surgery on the day of our consultation, on the day of our preoperative visit, and again on the day of surgery. I show patients in the mirror what to anticipate based on our discussion. Some patients want an upturned nose, some want a straight profile, some prefer to raise the tip of the nose and some do not. I do what we agree on before surgery. Despite my efforts, not every patient can be made happy. Patients who are sure about what they want and do not want, communicate to me their goals well, and have realistic expectations are typically happy with their results.
  Perception of a good or bad result after rhinoplasty is very subjective. Therefore, anyone considering rhinoplasty should look at the before and after photos and decide for themselves if they like the results.

Dr. Grigoryants’ Post-rhinoplasty Instructions

The following instructions are given to Dr. Grigoryants’ patients after rhinoplasty. These instructions may be modified for each patient depending on what was performed during surgery.

First day after surgery:  Keep you upper back elevated 30-40 degrees.  Apply cold compress (cloth soaked in cold water or crushed ice  in a glove placed over a cloth) during the first 24 hours. Avoid applying anything frozen directly on the skin.  Start taking antibiotics (prescribed by Dr. Grigoryants) the same evening and prescribed pain and nausea medications as needed (read instruction labels on the medications).

Facial swelling tends to get worse during the first 3 days after surgery and improves by the fifth day after surgery.  Call Dr. Grigoryants’ office to schedule your postoperative appointment 5-7 after surgery.

Starting day 3 : Start cleaning the bottom of the nose gently using cotton balls or facial tissue soaked in warm water.  Apply Vaseline on the  rims of the nostrils once a day. Do not put Q-tips inside the nose.

Day 5-7:  Dr. Grigoryants will remove your splints and sutures if nostrils were narrowed.  Having the nostrils lubricated with vaseline prior to the appointment makes suture removal less uncomfortable.  Dr. Grigoryants does not use nasal packing.

– Irrigate the nose with saline spray (also known as Ocean spray).  Apply Vaseline inside the nose at night for two weeks using a corner of an open paper napkin rolled into a cylindrical shape (Q-tips are too stiff and my cause injury ).  Sutures and dry secretions/dry blood will come out. Breathing will improve.

– Tape your nose only at night.  Although taping will not affect the final result, it will help minimize  swelling that develops overnight during the early postoperative period.

– Avoid alcohol and  blood thinners (aspirin, motrin, advil, excedrin, naproxen, ibuprofen, vitamin E, fish oil, omega 3 fatty acids, herbal  supplements ) for 2 weeks after surgery.

– Showers, make up, facial creams can be restarted.

Nose blowing: Can be started gently 2 weeks after surgery.

Driving:  You may drive when facial swelling is not impairing vision and you are no longer taking pain medications.

Exercises:  Avoid bending for 2 weeks after surgery.  Exercises can be resumed 1 month after surgery.

Swimming:  Swimming with submerging head in the water can be started 2 months after surgery.

Sun exposure: You may be in the sun but avoid prolonged exposure. Use a hat to protect your nose.

Pore strips:  May be resumed 3 months after surgery.

Unpleasant smell in the nose: May occur in some patients after rhinoplasty and will go away over time.

Stiff upper lip and smile:  Usually resolves by 2-3 months after surgery.

Flying on airplanes:  You may fly 7 days after surgery. Dr. Grigoryants will review special in-flight care instructions with you.

Nose surgery (nose job) can produce a very pleasing  aesthetic  balance of the face especially in  patients with large noses preoperatively. Common changes performed in rhinoplasty in such patients  are reducing the dorsal hump, refining and possibly raising the tip, and shortening the projection of the nose.   In some patients, the size of the nostrils is also reduced.

Aesthetic benefits achieve with rhinoplasty are the following:

-Reducing the dorsal hump allows to accentuate the eyes and cheek bones.

-Decreasing the projection of the nose improves the balance between the nose and the chin ( and lips)

The angle between the columella and upper lip is considered aesthetically desirable  to be 95-100 degrees in women and 90 -95 degrees in men.   This results in a more youthful appearance of  the nose in patients with droopy noses preoperatively

 

 

These are general guidelines. Patients’ preferences need to be considered when formulating a rhinoplasty plan.  For example, some patients prefer to preserve ethnic or family features and may not wish to reduce the size of the nose much. A combination of surgeon’s recommendations and patients goals typically results in a better nose reshaping balanced plan.

This post will describes what steps we take to ensure safety for our patients. When these guidelines are followed, plastic surgery can be performed with a very high degree of safety.

1. Patient selection:   Prior to surgery, patients are screened for any medical conditions and their ability to withstand surgery and anesthesia. For young and healthy patients, surgery is very safe. Older patients and patients with medical problems will need to undergo medical clearance by their primary care doctors or cardiologists. In certain cases, even stress tests will be ordered to evaluate the patients cardiac function. From time to time, we will turn down patients based on their age and/or medical condition.

2. Type of surgery performed: Certain procedures like (rhinoplasty, facelift, breast augmentation etc.) are associated with relatively little bleeding and little stress on the body. On the other hand large volume liposuction (over 5 liters) or tummy tuck with a large volume liposuction is more stressful for the body. When patients undergo combined surgeries or large volume liposuction, we recommend the patients stay in the hospital for at least 1 day for observation and intravenous hydration. During surgery surgery the patients vitals signs and fluid maintenance is very important. Dr. Grigoryants’ training and experience in Critical Care Medicine in addition to plastic surgery, allows him to take extra steps in ensuring intraoperative safety for his patients.

3. Duration of surgery: Most procedures in plastic surgery when performed by an experienced surgeon are relatively short. For example, Dr. Grigoryants’ rhinoplasty takes about 2-2.5 hours, breast augmentation 1.5 hours, and tummy tuck with liposuction 3-3.5 hours. Certain procedures can be combined safely. Although the relationship between the risk of complications and duration of surgery is not very clear, Dr. Grigoryants limits the duration of surgery to less than 6-7 hours when procedures are combined. When more extensive procedures are required, it is best to do them separately at different times.

4. Surgical Enviroment: Surgical procedures with anesthesia have to be performed in the hospital or an accredited surgical center. These facilities will have the necessary equipment and medications to allow for safer surgery. For example, patients vital signs are carefully monitored before, during, and after surgery; warming blanket are used to keep patients warm; and sequential compression devices (SCD’s) are placed on all patients to minimize the risk of deep vein thrombosis.

Combining rhinoplasty with other surgeries

Rhinoplasty is frequently combines with other procedures in our practice.  Patients undergoing rhinoplasty will undergo another procedure such as breast augmentation, liposuction, breast reduction, lip augmentation, ear reshaping or labioplasty.  The advantage of combining surgeries is one anesthesia, one recovery period, and lower cost as compared to doing these procedures separately.  As long as the total duration of surgery is not excessive, the procedures can be safely combined.  The duration of surgery depends on the surgeon’s skill and complexity of surgical problem.   For example,  rhinoplasty and breast augmentation with Dr. Grigoryants takes about 3- 3.5 hours. How Nose Reshaping Redefines your look

Nose reshaping is one of the most personalized surgeries that people want to pursue. Some people have a large hump on their nose, and some people have a bulbous nasal tip that makes their whole face look out of proportion.  Then there are some that suffer from chronic breathing complications, because their nose is disrupting airflow.   1. Get lab testing or medical evaluation to determine if the body is healthy for surgery

  2. Take medications or stop certain medications to avoid surgery complications

  3. Avoid taking aspirin, anti-inflammatory drugs or herbal supplements to avoid increase of bleeding

  4. Stop smoking in advance of surgery

There are also chores that need to be done at home to ensure a successful recovery in the weeks post-operation, which include:

  1. Stock up on medication or painkillers

  2. Stock up on ice packs or ice bags to apply for soreness

  3. Have ready made meals for after the operation (Soup, juice, soft foods, snacks)

  4. Wash your face thoroughly with antibacterial soap to eliminate germs

  5. Avoid use of makeup and hair products on surgery date

  6. Dress in loose, comfortable clothing the day of surgery

  7. Do not eat until after the procedure

  8. Have a driver available to take you home after the operation

  9. Arrange for a caretaker for two days, post-surgery

  10. Plan to take off work or school for at least a week to recover

If the patient becomes sick or catches a minor illness in the week or on the day of the surgery, call your surgeon to reschedule for a later date. Being sick can increase the risk of complications during surgery.

The nose takes time to settle into its new shape, so allow the body to rest as much as possible. Avoid strenuous activities for two to six weeks and try to avoid sun exposure for at least eight weeks.
